Welcome to the home page for the online forms required by Archdiocesan Interscholastic Athletic League (AIAL) and your school.

We would like to thank you for taking part in our process of accepting athletic participation paper work online. This automated process will allow us to be more efficient in handling the forms as well as saving valuable resources. These forms are required each year your son or daughter wishes to participate in athletics for in the AIAL. The Athletic Department would like to thank you and your son/daughter for filling out the forms and submitting them in a timely manner.

1. The required annual athletic physical and medical history cannot be filled out online because it must be completed by the parent/guardian and physician. This form must be on file at your school each year.

2. All other required paperwork will be done electronically. The parent/guardian will be asked to provide information such home phone numbers, cell phone numbers, email addresses, name of the primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically.

3. Remember that to submit online appropriately, you and your child must and sign in all required spaces. Forms will not be accepted electronically without all required information being completed.

4. To access the required athletic participation forms for the AIAL, hold your cursor over the “Electronic Participation Forms” tab. You will get a drop list of all required forms. 5. To begin, click on the form name and fill out the information requested. Parent/Guardian must have their son/daughters present to complete the forms. If you do not know the answer to a question, please answer the question with N/A, or none.

6. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create and “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an IPAD, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus.

7. Once you have filled out all of the information on each page, you will have the opportunity to print the document.

8. You will receive a confirmation email once the document(s) have been reviewed by staff at your school.

9. If your son/daughter requires the use of an inhaler, the Asthma Action Plan form must be completed by the treating physician. You may print this form by selecting the Printable Forms Tab at the top of the page.
